DCHD moved into Phase 1B
We have pre-planned recurring vaccine clinics scheduled weekly through February.
When we receive an update on the status of vaccine shipment for clinics the first week of February that information will be shared. We currently expect to receive the vaccines allotted for clinics on February 3rd and February 4th sometime between 1-5pm on January 29th. Upon accepting shipment of the vaccines, IDPH will send a link for pre-registration. DCHD will share this link on our website immediately following.
Vaccine delivery date determines what day we receive & share the pre-registration link. Upon receiving the shipment for our next clinic, a link will shared.
The first of our pre-planned recurring public vaccination clinics for Phase 1B (65 and over ) 1/27/21 and 1/28/21. Weekly clinics have been tentatively scheduled each Wednesday and Thursday presuming the number of vaccines allocated to DCHD are sufficient to proceed with clinics. When we learn knew information, it is shared with the public after. Thank you for your patience.
When a pre-registration link is sent to DCHD, we share the link here on our website. Once pre-registration is open, it will be in a large red box that says Pre-Registration for 65 & Older is now open. Individuals will click on the link of their preferred weekly clinic location. Upon opening the pre-registration link, after selecting an available clinic date/place, you will select 1 ticket and then enter personal information. The personal information requested is available on this form. VaccineClinic_PreRegistrationTemplate_COV19
Please remain patient. We understand our supply of vaccines does not meet the demand.
We are doing all we can to vaccinate those interested & eligible in Phase 1B as safely, quickly, and as fairly as possible.
Thank you for your continued patience.

Important Announcement:
Thank you to all who utilized our COVID immunization survey to indicate your interest in receiving the Moderna vaccine. Due to the sheer volume of respondents, and the enormous increase in those eligible for Phase 1B, the survey had to be closed to new entries. Its intended purpose was to gauge community interest in vaccination and provide DCHD with a pool of names to call on if there were leftover doses from opened vials as we vaccinated Phase 1A recipients. That was achieved.
If you are 65 & Older, please watch this site on Friday’s between 1-5pm to pre-register for a vaccine appointment at our next weekly clinic. For the first few weeks of February, vaccine clinics will be closed pods only – meaning that registrations are limited to those 65 & Older. While Phase 1B does include eligibility for other groups, the amount of vaccines currently being allocated to DCHD is a mere fraction of the number needed to vaccinate all eligible for Phase 1B.
DCHD will attempt to reach the interest survey respondents through Hyper-Reach, its mass notification system, to notify of upcoming clinic dates and times. Respondents will receive a phone call from phone number 217-599-0012 but clinic registration is currently only open to those 65 & Older for the first several weeks of February. DCHD will update the public when additional priority groups who are Phase 1B eligible can pre-register for an upcoming clinic.
